Martin’s Point is developing a team approach to providing care. In addition to the primary care physician, members of this team include a dedicated nurse, medical assistant and the person who answers the phone when patients call. We believe this approach will provide a more personalized experience for patients, improve the coordination of their care and ultimately yield better health outcomes.

Pathways To Excellence Blue Ribbons

The Maine Health Management Coalition’s Pathways to Excellence Primary Care program awards up to three blue ribbons to doctors’ offices across the state that provide high quality health care. Martin’s Point Health Care Centers all received multiple blue ribbon distinctions for diabetes care, cardiovascular disease, and for efficient office systems.

Martin’s Point believes that in order to provide the best possible health care experience, physicians and other clinical providers must be supported in their process of delivering care.

We are working to transform the way health care is delivered and paid for in ways that benefit patients, providers, and the community at large.

Developing a New Model of Delivering Care

Martin’s Point is leading the way by developing a new physician-led, team-based model for delivering care in our Health Care Centers throughout Maine and New Hampshire. This team-based model will enhance the patient experience by providing patients with easier access to care, better coordination of care, and the opportunity to feel like they are partners with their doctors and other clinical providers in managing their health.

The model also provides benefits to providers. All care team members who interact with patients work up to the maximum level of their training. Thus, the model allows providers to have richer quality time to spend with their patients.

Supporting Our Network Providers

In our role as a health plan administrator, Martin’s Point works closely with thousands of primary care physicians and specialists throughout the Northeast, acting as a responsive partner in the care they provide.

Health care in our country has become a system of “hand-offs,” where a patient is transferred from one provider to another, sent in and out of hospitals, and referred to ancillary services. In order to improve care and reduce risk, we need to better manage these patient transitions. Some ways we do this include utilizing care managers and technology, such as improved use of electronic health records and population health reporting.

We are also working on a project to redesign our primary care payment structure, paying physicians based on quality and overall cost of care, as well as for patient satisfaction.

Through this work, we hope to create an environment that not only improves the patient experience but also the satisfaction levels of physicians and other clinical providers.
